Registration

Applications must be filled out on site. There is a $50 registration fee and two weeks deposit if paying weekly OR one month's tuition if paying monthly. This will be adjusted with the last 2 weeks or last month of tuition. In addition to the application form (your contract with us), we will need an up to date immunization record and other State mandated forms that are available at our office.

 

Children must complete immunization requirements and be in good health. Children will not be admitted into our program without a medical record of shots & recent physical.
